Athletic Eligibility Packet Collection

The eligibility packet must be completed prior to participating in summer camps, tryouts, or practices.  To ensure the student-athlete is cleared for all seasons of sport for 2024-25, Cypress Athletics recommends the physical be dated May 13, 2024 or after.  This will prevent your student-athlete from potentially having his/her season interrupted because of eligibility.

Cypress Athletics will be using homecampus.com for Athletic Clearances for the 2024-2025 school year.  NO paperwork will be submitted to the athletic department.  All paperwork will be uploaded to the homecampus.com website, and will be verified online by the athletic directors.

RETURNERS: Your families will go back to their HomeCampus.com accounts and start a new clearance for 24-25.  Their student and parent information will re-load from drop-down menus.  They will have to re-do things like their medical history and sign documents, and upload new physical

In order for a student to participate in the summer programs, they must have their eligibility packet cleared.

Students must be cleared through the athletic department before participating in tryouts, practices, or games, and must carry a 2.0 gpa.

If students are getting a physical from Kaiser, Kaiser must stamp the AUHSD physical form, and may electronically sign their "full activity; cleared for sports" form by the M.D.
